Home Health-FT/PRN (East Central- East Houston, Baytown, Channelview) (Southeast-Pasadena, League City, Pearland, Friendswood, South Houston) This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.Provides services that are ordered by the physician as indicated in the plan of care, as developed, delegated, and supervised by the physical therapist. Notifies supervisor of changes in patient status or untoward patient responses Completes appropriate documentation in a timely manner.Administers non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities to patients. Discontinues any treatment which in their judgement is harmful to the patient.Applies concepts of infection control and standard precautions in performing patient care activities to prevent contamination and transmission of diseaseAccepts clinical assignments that are consistent with education and competence to care for patientsUses effective interpersonal relations and communication skills to provide patient and caregiver education.Meets mandatory continuing education requirements.Promotes compliance with all fiscal intermediary and/or other third-party payors, through education, coaching, and other assistance, as necessary.
