We are seeking a Nephrology trained physician to join us in pursuit of high-quality patient care for patients with both chronic and acute kidney disorders.

- One to three years of nephrology clinical experience in an outpatient setting.
- Ability to diagnose and treat a wide variety of kidney conditions to improve patient outcomes.

- Excellent communication skills.
- Interest in Population Health Management.
Responsibilities:
- Conduct comprehensive exams to evaluate kidney function for disease and disorders.
- Treat a wide spectrum of diseases across the entire continuum including high blood pressure, fluid retention, complex metabolic conditions, electrolyte, acid-based disturbances, mineral imbalance and infections with detailed attention to identify abnormalities and kidney neoplasms.
- Collect and record patient information, such as medical histories, reports and examination results. Maintain complete and accurate medical records.
- Order, analyze lab results, and interpret image studies with strong understanding of various medications that affect kidney function.
- Prescribe medications and formulate ongoing treatment and disease management plans.
- Provide relationship-centered care, considering the patient’s psychosocial and physical needs.
- Collaborate with a multidisciplinary team to develop and implement patient care plans.
- Communicate effectively with the clinical team to ensure total delivery of quality care.
Qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
- Completion of four year post graduate medical training from a U.S. accredited residency program by the Accreditation Council for Graduate Medical Education or American Osteopathic Association (AOA) in Internal Medicine.
- Completion of two-year fellowship training in Nephrology
- Board Certified in Internal Medicine
- Board Certified or Eligible in Nephrology Medicine
- Must possess current, unrestricted New York State license.
- Must possess active Drug Enforcement Agency (DEA) registration or be eligible for registration.
- Must possess active Medicare/Medicaid Enrollment or be eligible to enroll.
- BLS or ACLS required.
- Must possess strong communication skills both oral and written.
- Comfortable with Electronic Medical Record (EMR) system and Microsoft Office.
