A education network in New York City is actively seeking a licensedSpeech Language Pathologist (SLP) to join one of their OPWDD preschools in a Remote role.
About the Opportunity:
- Setting: Article 16 Clinic
- Schedule: Monday to Friday
- Hours: 9am to 5pm or 8am to 4pm
- Patient Population: Adults (Ages 18+)
- EMR: PrecisionCare
Responsibilities:The Speech Language Pathologist (SLP) will:
- ReviewMDs referral and persons receiving services medical record prior to determining speech/language therapy services
- Evaluateand assessthe persons receiving services needs and level of functioning in the areas of: speech, language, cognitive & social aspects of communication, voice, augmentative and/or alternative communication (ACC), and eating /management of diet (dysphagia)
- Devisetreatment plans based on the assessment; administer treatment involving the application of communication and/or feeding aid modalities
- Consultwith supervisor when appropriate to implement recommendations
- Provide training to persons receiving services, families, direct care and/or outside agencies, if warranted
- Communicate effectively with discharge planning from rehabilitative programs
- Participateand attendcare plan/team meetings for all persons receiving Speech-Language Therapy services
- Ensure all documentation is completed through EMR within the time frame established
- Perform other duties, as needed
Qualifications:- Bachelor's Degree
- NYS Speech Language Pathologist (SLP) license
- TSSLD
- Solid assessment and documentation skills
Desired Skills:- 1+ year of Clinical experience
